When caring for a loved one with complex needs, communication can be one of the most complicated yet essential aspects of daily life. So much information lives in our heads: routines, preferences, triggers, comforts, medications, and moments that matter. Yet that information doesn’t always transfer easily between caregivers, staff, educators, or support teams. Trying to remember everything was not practical.
That gap is where the About Me book was born.
The Challenge We Kept Seeing
Families were repeating the same information over and over:
• Explaining behaviors that weren’t always understood
• Reintroducing their loved one every time staffing changed
• Worrying that important details would be missed in moments of transition
Even the most well-intentioned teams can’t know everything, especially when care is shared among multiple people and across various environments.
Families needed something consistent. Something human. Something that spoke for their loved one when they couldn’t.
What the About Me Book Is
The About Me book is a personalized, living resource that helps caregivers, educators, and support professionals better understand the person they are caring for—beyond diagnoses or charts.
It includes space to document things like:
• Daily routines and preferences
• Communication styles and sensory needs
• Triggers, calming strategies, and comforting items
• Medical and behavioral notes
• What brings joy, confidence, and connection
Most importantly, it centers the person—not the paperwork.
How This Tool Supports Better Care
The About Me book helps by:
• Reducing misunderstandings during transitions
• Providing continuity when staff or caregivers change
• Supporting calmer, more respectful interactions
• Helping teams respond with patience and perspective
When caregivers understand why something matters, support becomes more effective—and more compassionate.
A Living Document, Not a One-Time Form
This book isn’t meant to be filled out once and set aside.
It evolves as your loved one grows, preferences change, and life transitions occur. Families update it regularly—adding notes, revising routines, and capturing new insights over time.
Think of it as a trusted voice that grows alongside the person it represents.
Who This Resource Is For
The About Me book is helpful for:
• Families navigating group home transitions
• Individuals receiving support across multiple settings
• Care teams seeking better communication and consistency
• Anyone who wants care to feel more personal, informed, and respectful
